Vice President, Provider Strategy & Population Health
Fiona Weber serves as Vice President of Provider Strategy & Population Health at Town Square Health, where she leads initiatives to strengthen clinical partnerships and advance population health performance across the organization’s value-based care delivery model. In her role, Fiona focuses on aligning provider strategy with population health goals, improving care outcomes, and developing scalable best practices that support high-quality, patient-centered care for Medicare-eligible populations.
Fiona brings extensive experience in population health, performance excellence, and strategic operations in healthcare. Before joining Town Square Health in 2025, she spent more than seven years at Oak Street Health, a leading value-based primary care provider, where she held progressive leadership roles—including Vice President, Senior Director, and Director of Population Health—driving initiatives that improved clinical outcomes, reduced costs, and strengthened cross-functional collaboration. She also served on the Executive Advisory Board at Medallion, contributing expertise on healthcare transformation through the use of technology. Earlier in her career, Fiona worked in business development and operational roles at CHI St. Luke’s Health and supported health systems and academic projects as a graduate consultant.
Fiona holds a Master of Healthcare Administration from the University of Minnesota, where she focused on health systems performance and leadership, and earned a Bachelor of Science in SUNY Oneonta.
Throughout her career, Fiona has demonstrated a passion for transforming healthcare delivery through evidence-based strategy, operational excellence, and collaborative leadership that advances equitable care for all communities.
