David Buchanan, MD, MS
Co-Founder and Chief Executive Officer
David Buchanan, MD, MS, is the Chief Executive Officer of Town Square Health, where he leads the strategic vision and execution of a patient-centered, value-based care model designed to improve access, quality, and outcomes for Medicare-eligible populations. Under his leadership, Town Square Health unites primary and specialty care around patients’ needs, emphasizing coordinated, preventive, and longitudinal care.
Dr. Buchanan is a board-certified internist and seasoned healthcare executive with extensive experience in value-based care and clinical operations. Prior to founding Town Square Health in 2025, he served as the Chief Innovation Officer for CVS Healthcare Delivery. In that role, he led strategic efforts to improve health outcomes and organizational performance at Oak Street, Signify Health, and other CVS Healthcare Delivery businesses. Previously, he served as the Chief Clinical Officer at Oak Street Health and other leadership roles for over seven years. Prior to that, he held leadership positions at a community health center, an Medicaid ACO, a national Health Information Technology Organization and a public hospital system, helping build care models that improve health equity and reduce avoidable hospitalizations.
Dr. Buchanan earned his Bachelor’s degree in Economics from MIT, his MD from the University of Chicago Pritzker School of Medicine, and completed his internal medicine residency at the University of California, San Francisco (UCSF). He also holds a Master of Science degree, further strengthening his ability to integrate clinical insight with operational leadership. A fellow of the American College of Physicians, he continues to practice medicine and contribute to research and academic programs focused on health systems innovation.
